Consent to the processing of personal data
I acknowledge that by filling in and sending the form, I express my consent to the processing of my personal data for inclusion in a database established and managed by the company Domalewski Advisory s.r.o., ID number 047 11 734, with its registered office Revoluční 724/7, 110 00 Prague 1, as a controller of personal data.
I am giving consent according to the relevant legal regulations for the processing, storage, collection and treatment of personal data (hereinafter referred to as "processing of personal data") for the purpose of sending business and marketing offers. These may be my personal data: name, surname, telephone number, e-mail address, data regarding our communication and all other personal data provided by me. I give this consent for an indefinite period and I am entitled to withdraw my consent to processing of personal data at any time by sending an e-mail to: info@d-advisory.com.
This consent is given in accordance with Article 6 (1) (a) Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation). I declare that I have been informed by the controller about my rights, so I am aware that I have the right to ask the controller for information about the processing of my personal data, about which data are processed, the right of access to my personal data, the right to request the controller for clarification, rectification, updating and erasure of my personal data, the right to ask data portability, the right to raise objections to the processing of personal data and, in case of doubts about compliance with the obligations related to the processing of personal data, the right to contact the controller or the Office for Personal Data Protection.
In addition to the above, I acknowledge that the controller may entrust the processing of personal data to a third party.
